FORMAT FOR ADMINISTRATOR ACTION PLANS 

SECTION I: INTRODUCTION

This is to be a one-page listing of the information below:

  • Name
  • Address
  • Social Security Number
  • Educational Background:
  • Degrees held
  • Professional Background:

– Number of years in education

– Positions held, including present position

  • Current Certification
  • Certification to be Renewed, date of expiration
  • Mentor

– Name, Position, Business Address, Phone Number

  • Date of Submittal

SECTION II: SUMMARY OF WORK COMPLETED SINCE LAST CERTIFICATION

SECTION III: CHART

In order to be certified for the first time, Maine administrators are required to demonstrate a “basic level of knowledge” in competency areas described in the certification law. Thereafter, in order to renew certification, administrators are expected to demonstrate they have completed 90 hours in any of the 10 Professional Standards for Educational Leaders.

Indicate the number of hours obtained in each standard on the chart: Professional Standards for Educational Leaders.

SECTION IV: MENTOR REVIEW AND VERIFICATION

Mentor reviews Plan and signs proposed or completed portfolio form (as appropriate) to indicate that the Administrator Action Pan has been reviewed and is ready for submission to the Consortium.

Administrators have the opportunity to demonstrate a commitment to rigorous professional development for themselves and their profession. All signs indicate the majority of Kennebec Valley administrators are ready to embrace that opportunity.

NOTE: Recertification Plans, once approved, will be retained by KCSA. Please do not submit originals of important documents that you may wish to have available for other purposes.
